The form contains blocks that restrict the ability to select certain data. If it cannot be properly filled, it means that it does not apply to the damage in question. (e.g. if the accident was caused by a Polish vehicle and took place in Poland).

If the accident took place in Poland and was caused by a vehicle with Polish registration, the MTPL insurance of the accident’s perpetrator may be verified on the Polish Gurantee Fund website here
